Consent given through this form is effective during the current school year unless revoked earlier.
In accordance with law, the District must provide parents with written notice of each school-based health-related service offered at the campus their child attends. These routine services promote student safety, wellness, and readiness to learn. The services may be provided by qualified school staff, including nurses, CNAs, and athletic trainers.

Colorado ISD Statement on CMS Campus Situation
Just before lunch today, an incident occurred at the Colorado Middle School campus involving our long-time substitute teacher. During the situation, our substitute fell and sustained a head injury.
Students and staff responded immediately. Our district nurses provided care until Mitchell County EMS arrived on the scene. Our substitute was transported to Mitchell County Hospital, and we are currently awaiting updates on his condition.
This happened when students had been released from class for lunch. Out of an abundance of care, our District Mental Health Counselor is with students who were upset by this situation, ensuring they have support during this time.
Our thoughts and prayers are with our amazing substitute, and we are all hoping for a full and speedy recovery.

A note is being sent home with students today for grandparents who plan to join our students for Grandparents Day on September 5, 2025.
